Pre-admission is an important part of your pre procedure care. Your specialist will explain your procedure and complete the consent form with you during your consultation.
IVF patients will complete consent on the day of surgery.
If you have any questions about your planned day procedure admission, the completion of forms, costs or health insurance status, please contact our team on 03 9088 0494.

Pre-admission form

To ensure we can confirm your admission, health insurance and other details, please complete our online preadmission form at least 7 days before your admission.

Pre-admission assessment

Nursing staff will call you two day prior to your admission and provide vital information for your admission and conduct a patient assessment.
Please notify us if your weight is over 130kg prior to your admission as we will need to organise a pre-admission assessment with your Doctors.

Private Health Insurance

If you have private health insurance, please contact your health fund to check your level of cover for hospital admission.
We recommend you ask the following questions:
  • Am I fully covered for an admission in a private hospital?
  • Do I have an excess or any co-payments associated with my policy that I need to pay?
  • Have I served all current waiting periods?
  • Are all my premium payments up to date?
  • Does my policy have any restrictions, limitations or minimum benefits that may apply to my planned procedure?

Reminder

  • If you have recently changed your health fund, or changed your level of cover with the same health fund, does this new change cover this admission and procedure?
